Pre Authorization / Pre Certification

Many insurance companies require a referral and/or pre-certification (especially for CT and MRI), it is the responsibility of the patient and referring physician to obtain approval in advance in writing. Failure to obtain the required information may result in non-reimbursement or delay in treatment.

Empire Contract Expiration for JFK Advanced Medical Imaging

Please be advised that effective December 1, 2018 JFK Advanced Medical Imaging and Empire HealthChoice HMO, Inc. and Empire HealthChoice Assurance, Inc. will be ending their contractual relationship.

Hackensack Meridian Health still participates with a variety of insurance plans. Patients seeking information on accepted Hackensack Meridian Health insurances can visit

Empire patients with out-of-network benefits may opt to receive care at non-participating facilities. However, use of JFK Advanced Medical Imaging and other Hackensack Meridian Health facilities may trigger additional patient financial responsibility.
